Housing for Health: Data and Program Innovations from NYC Health + Hospitals

Amidst rapidly-growing interest in how health systems can work to address social determinants of health, NYC Health + Hospitals has several initiatives to improve health care access and pathways to housing for patients experiencing homelessness.

At this seminar, we will hear from Laura Jacobson, MSPH, Director of Data Services in the Office of Population Health and Fionnuala Seiferth, MPA, Director of Housing Policy and Operations on the Housing for Health team, both at NYC Health + Hospitals. The event will be moderated by Patricia Hernandez, LCSW, Director of the Metro Region for the Corporation for Supportive Housing.

Presenters will discuss how NYC Health + Hospitals identifies patients experiencing homelessness and uses data and dashboards to foreground their health needs and plan targeted programs. We will also learn how the NYC Health + Hospitals Housing for Health initiative works to facilitate permanent housing placements for patients experiencing homelessness.
